Reached 34 new seniors with Alzheimer’s Disease and Related Dementias (ADRD) and their family caregivers in North Hudson, NJ forin-person assessments, remote coaching, treatment planning, education and legal overview.
Conducted a total of 117 Alzheimer’s Care Project discussions in person, via phone, or virtual format with new clientele.
Increased ADRD diagnosis by approximately 63%.
